Advance care planning is about planning and communicating your wishes in relation to care at the end of life.

End of life care

Care at the end of life is available for everyone regardless of age, culture, background or beliefs.

There are many types of care and support available to assist patients, family members and carers through all stages of a terminal illness.

If you are living with a life limiting illness it is important for you to understand your options for treatment and care and make your wishes and preferences clear.

We can assist in ensuring that your wishes and goals are listened to, are realistic, and that appropriate care follows.
