Hampstead Centre is launching a new program: CO-ACT (Care of the Older Person and Community Transition), focused on providing best-possible-care outcomes for older persons transitioning from acute care to ongoing care.
We’re looking for new recruits as we establish our model of care at the Hampstead Centre. The CO-ACT team will apply new ways of working that provide the best possible care for older patients transitioning towards aged care.
Our CO-ACT team at Hampstead works together closely and collaboratively; we’re like a family. You’ll join a supportive organisation, committed to best practice where your ideas and feedback will be valued.
